Spain's Sistema Nacional de Salud (SNS) is financed federally through general taxation but managed by the 17 autonomous communities (e.g. SERMAS in Madrid, CatSalut in Catalonia), and is internationally recognized for its transplant surgery, oncology, and preventive care. Access via the health card (Tarjeta Sanitaria Individual) is automatic for legal residents contributing to Social Security (employed/self-employed), state pensioners, and DNV holders paying local taxes — registration requires the social security affiliation document and a municipal residency sheet (empadronamiento) at the local Centro de Salud. For legal residents not contributing to Social Security (e.g. Non-Lucrative Visa holders), the Convenio Especial pay-in option is available after 1 full year of continuous empadronamiento, at a fixed 2026 premium of €60/month (under 65) or €157/month (65+).
